The standard advice for a blocked bowel is the same everywhere: more fibre, more water, regular habits, more movement. It is reasonable advice and it helps many people. It also fails a predictable subset — and in Eight Constitution Medicine (ECM) that subset is not random. This article is about constipation and constitution, and about why the same remedy can resolve the problem in one person and entrench it in another.
In Summary
- Medically, constipation means fewer than three movements a week, straining, hard stool, or a sense of incomplete evacuation, persisting three months or more. It affects roughly 5–20% of the population, more often women and older adults.
- In ECM the cause differs by constitution, so the remedy has to differ too.
- Aloe cools and moistens the colon and lower abdomen — suited to the hot, strong-digesting Earth types; poorly suited to the Wood types and to the Water types.
- Green juice is the natural choice for the Gold types and the wrong one for the Wood types, who should be worrying about loose stool instead.
- Honey moistens the intestine and restores strength — helpful for the Water types; it can worsen constipation in the Earth types.
- Cold water suits the Earth types specifically. Lukewarm water suits everyone.
- The most overlooked cause is not food type but food quantity. Older adults and the Water types often constipate simply because they are not eating enough.
- Before treating constipation, establish whether the person is eating properly. Giving aloe to someone with no appetite makes both problems worse.
In ECM, “strong” does not mean “safe” — the dominant axis carries the most qi(気) and tips most easily into excess. It is worth stating here because two of the constipation patterns below arise from an organ system being overactive rather than underactive.
Why Constipation Is Worth Taking Seriously
It is easy to file constipation under minor discomfort. It deserves a little more than that. Chronic constipation has been reported in association with headache and with mood disturbance, and there is discussion in the literature of associations with cardiovascular and cerebrovascular risk. These are reported associations rather than demonstrated causation, and I would not press them further than that.
The mechanism I find more directly useful is the one about clearance. Waste that sits in the bowel for a long time gives its breakdown products more opportunity to be reabsorbed, and skin conditions — acne, atopic flare — often move in step with bowel function. I have seen skin improve when constipation resolved often enough to take the connection seriously, though I would present it as a clinical observation rather than a settled mechanism.
Where the Standard Remedies Actually Land
What follows describes how these foods behave in ECM terms. It is not a recommendation to take any of them; decisions about what to take, and when, belong with the physician treating you.
Aloe
Aloe cools and moistens the large intestine, the lower abdomen and the kidney.
- Suited to: Pancreotonia and Gastrotonia — the constitutions that run hot and digest strongly. It can also help Pulmotonia and Colonotonia.
- Caution: Hepatonia and Cholecystonia already have a cold lower abdomen. Long use may relieve the constipation briefly and then leave the colon weaker, with constipation and diarrhoea alternating. For Renotonia and Vesicotonia it can be actively unhelpful.
Green juice
- Suited to: Colonotonia and Pulmotonia. If a Gold type is constipated, this is often the most natural correction available.
- Avoid: Hepatonia and Cholecystonia. These are the two constitutions in which constipation is least common in the first place — their risk runs toward loose stool. Even when a Wood type is constipated, green juice is the wrong instrument.
Water, and its temperature
Drinking water helps constipation regardless of constitution. Temperature is where the constitutions separate.
Cold water suits Gastrotonia and Pancreotonia, and the reason is worth spelling out because it is often misread. Cold does two things: it cools heat, and it drives qi downward. In the Earth types both actions are useful — the stomach fire comes down and the qi is pushed toward the recessive kidney. That is why the general rule about avoiding cold food does not extend to the Earth types.
Lukewarm water is the safe default for everyone else. Even hot water ends up settling heat and replenishing fluid once it is inside, so the hot-natured Earth types do perfectly well drinking it warm rather than iced.
Honey
Honey moistens the intestine and restores strength.
- Suited to: Vesicotonia and Renotonia, and to anyone whose reserves have dropped substantially. Dissolved in water or taken with food.
- Caution: in Pancreotonia and Gastrotonia honey can make constipation worse.
Vegetables and fruit
Fresh produce helps constipation in general — with one clear exception. In quantity, raw vegetables and fruit can cool or slacken the bowel in Vesicotonia, Renotonia, Hepatonia and Cholecystonia, and make the constipation worse rather than better. Cooked rather than raw is the usable form for these four.
The Two Causes That Are Not About Food Type
Stress, the liver, and a bowel that stops moving
KTM holds that stress accumulates heat in the liver. When the liver cannot discharge that heat alone, the large intestine is one of the organs that assists. The consequence is that liver heat leaves the colon rigid, its motion stalled and its internal fluid dried out.
This is why constipation appears during stressful periods, and it carries a practical implication: getting irritated about the constipation feeds the mechanism that is causing it. I have written about this pattern at more length elsewhere, and it is worth reading alongside this article.
Quantity — the cause most often missed
Very often the type of food matters less than the amount of it. Stool is made out of what you eat, and if not much goes in, not much is available to come out.
Older adults constipate frequently for exactly this reason: strength falls, appetite follows, and intake drops. Renotonia and Vesicotonia constipate the same way — naturally small appetites, and long intervals as a straightforward consequence.
This changes the order of operations. Give aloe to someone whose appetite is already poor and you will suppress it further, and the constipation will deepen. So before doing anything about the bowel, establish whether the person is eating properly. That single question redirects a surprising proportion of cases.
What to Do With All This
Defecation is not like exercise or work — it is not something you accomplish by conscious effort. Eat well, move reasonably, and it happens by itself. Most of what looks like a bowel problem resolves when the basics are in place.
Where the basics are in place and the problem persists, the constitutional question becomes worth asking: not what is the remedy for constipation, but which of these mechanisms is producing mine. That is a question worth putting to a KTM physician who can diagnose your constitution.
And as with any change in bowel habit: a persistent unexplained change, blood in the stool, unintended weight loss, or ongoing abdominal pain should be assessed medically rather than managed with diet alone.
Summary
Constipation is not one condition with one answer. It is at least four different mechanisms — a cold and slackened bowel, a hot and dried one, a stalled one, and an empty one — and the remedy that fits one of them will make another worse.
