Say Renotonia or Vesicotonia and most people picture something cold and easily tired — a chilly interior, a sluggish digestive system, someone who wears out at the slightest exertion. That picture is often accurate. But people are not that simple, and the exceptions cause real trouble. This article covers what happens with red ginseng and the water constitutions when the patient is already running hot.

The Standard Picture, and Where It Stops
Patients with weak stomachs and poor digestion frequently do have a cold, stiffened stomach. Vesicotonia in particular often shows gastroptosis — the stomach hanging low because it lacks the tone to hold position.
All of that is accurate as far as it goes. But a person is not so simple a creature.
Being Renotonia or Vesicotonia does not mean being uniformly cold and without energy. What I want to describe here is the dilemma faced by the water types who run hot, and what constitutional eating should actually look like for them.
Ambition Outrunning Digestion
Among the water constitutions there are a great many people with abundant drive. They want to do a great deal and their ambitions are large — and their innate digestive capacity cannot produce enough energy to fund it.
The mind runs ahead while the body lags, and the unease characteristic of these constitutions grows. What follows?
Empty heat (허열 虛熱) arises, and qi leaks away faster still.
Rest at this stage and the person recovers. Keep pushing on drive alone and it becomes illness: heat accumulating while body fluids dry out, digestion dropping further so that nothing can be replenished — a closed loop.
Which is why the patient presents looking, to anyone’s eye, like someone with a lot of heat — sweating, heat sensation, headache, disturbed sleep, constipation. And on examination they turn out to be Renotonia or Vesicotonia, considerably more often than one would guess.

Why Red Ginseng Is Not Automatically the Answer
Approach this mechanically — water types have cold bodies, so they should eat warming things — and the trouble starts here.
The classic example is red ginseng. It is known as a tonic for the water constitutions, and for the standard presentation that is reasonable. But give it to a water type whose heat is already up, whose chest is racing and who cannot sleep, and it is fuel poured on a burning house. Things get worse.
The same holds for herbal treatment. The basic formulas for Renotonia and Vesicotonia are largely built from herbs of warm nature. Applied unchanged to a patient complaining of heat signs — palpitations, skin disease, hwabyeong, severe headache — a constitutional formula may do nothing or may cause harm.
In such cases, in my clinical experience, prescriptions aimed at the presenting pattern work considerably better than the constitutional formula. Once the illness has settled and the aim shifts to building the body up over the long term, the constitutional approach becomes appropriate again.

Three Adjustments to How You Eat
Rather than following the “beneficial foods” column blindly, adjust the preparation and composition to the state your body is actually in.
1. Avoid heavy seasoning and heavy cooking
Even a food beneficial to the water constitutions becomes hotter in nature the more it is spiced, salted or thoroughly cooked.
Onion is a good food for water types — but the longer it is fried, the sweeter it becomes and the faster it is absorbed, which means it generates heat in the body more quickly and in greater quantity.
2. Eat things less cooked
Where heat is high, vegetables and other foods are better lightly blanched, or eaten in a less processed state, than thoroughly cooked or fried. The point is not to push the food’s own nature too far toward hot, so that it does not provoke the heat already accumulated inside.
3. Eating across the range may serve you better
If your body is currently so hot that it is distressing, there is no need to insist exclusively on the warm-natured foods listed for water types.
Eating across the range to obtain nutrition, favouring soft foods that digest easily, is frequently more helpful than a mechanical constitutional diet.
Watch What Your Body Does
Just as a person’s life is not decided by a single genetic factor, health is not settled by one page of a constitutional chart.
Even if you are Renotonia or Vesicotonia, if your body is burning right now, putting out the fire comes first.
Rather than keeping to the constitutional diet mechanically, what is needed is the flexibility to observe your body’s responses and find your own answer.
The core of it, if the rest feels complicated: heat on the surface does not mean heat all the way through, and the same constitution requires different eating according to the situation.
