Of the eight constitutions, Colonotonia is the one whose energy tilts furthest outward — and it is the one that registers a racing heart most readily. But the heat does not accumulate on the inside here, which changes what the symptom means. Colonotonia heart palpitations are less a warning about the heart than an instruction about attention.

The Constitution That Faces Outward
In Eight Constitution Medicine (ECM), developed by Korean physician Dowon Kuon in the latter half of the twentieth century, Colonotonia runs lung and large intestine > kidney and bladder > pancreas and stomach > heart and small intestine > liver and gallbladder.
Qi is concentrated in the lung and the kidney, and both of those tilt the flow outward. That single fact explains most of what follows.
These patients are interested in what is happening outside themselves and are good at noticing it. External change registers. Events register. And the skin is sensitive to a degree that, in my clinical experience, makes it reasonable to treat it as a sense organ in its own right — not a metaphor but a working description of how these patients take in their surroundings.
When the body tires, that responsiveness sharpens rather than dulls. Which is why Colonotonia has the reputation it has for being the sensitive constitution.
The Article That Preceded This One
The article on Pulmotonia in this series made the comparison from the other side. Both constitutions are Taeyangin, and both have a dominant lung. What differs is the second position: Pulmotonia’s is the digestive axis, which turns qi inward, while Colonotonia’s is the kidney, which keeps it pointed out.
So of the two metal constitutions, this is the one that receives the signal more often. Same category, different temperament.
What the Symptom Is Saying
A racing heart, in this framing, is a message: this is not a moment to rest easy — have a look around.
A constitution built to monitor the outside world will receive that message frequently, because it is attending to a great deal. Sleep disturbance arrives for the same reason and often at the same time.
Why It Does Not Become Chronic
Here is the reassuring part, and it follows from the hierarchy.
Energy does not accumulate in the interior of this constitution. The flow runs outward; there is no reservoir filling up inside. So while palpitations arrive readily, they do not entrench — sleep hygiene and attention to food are generally sufficient to manage them.
Turn that around and it yields something worth telling patients directly: being sensitive, or noticing your heart more than other people notice theirs, is not in itself a reason for much concern here. It is the constitution working as it is built to work.
This assumes, of course, that appropriate evaluation has been done. Palpitations that persist deserve conventional assessment — arrhythmia, thyroid disease and anaemia all present this way. The reassurance is available afterwards, not instead.
What Not to Do
Vigorous exercise that raises a heavy sweat. Colonotonia is sympathetic-tense, and sweating costs this constitution.
The four-and-four divide is worth restating here, because this series has produced the opposite instruction twice already. Hepatonia, Cholecystonia, Pancreotonia and Gastrotonia are parasympathetic-tense and benefit from dispersal — for them, sweat is treatment. Pulmotonia, Colonotonia, Renotonia and Vesicotonia are sympathetic-tense and are depleted by it. The article on Hepatonia recommends the sauna; this one recommends staying out of it.
When symptoms are pronounced, reduce meat. Meat, dairy, wheat flour and oil do not suit the metal constitutions to begin with. If meat is eaten, boiled preparations sit better than grilled or fried.
What Helps
- Quince tea
- Vegetable juices — cabbage among them
- Swimming — the exercise of choice for the metal constitutions
- A light, cool shower that stops short of raising a sweat
None of this is elaborate, and that is appropriate to a complaint that is not, in this constitution, structurally entrenched.
The Part That Matters Most
In my clinical experience, the instruction that does the most work for Colonotonia is not on the list above.
It is this: when the heart starts racing, the useful realisation is that it is time to reduce your interest in the world and rest.
For a constitution whose energy runs outward — that follows what is happening, notices what changes, registers what other people miss — the cost of that attention is real. The palpitations are the invoice. Turning down the volume for a while does more than any tea.
Summary
Colonotonia races because it is pointed at the world, not because heat is piling up inside. The symptom arrives easily and leaves the same way, provided the person takes the hint.
Which is why the prescription here is mostly a matter of attention rather than of treatment.
