A nosebleed is usually taken as the classic sign of overwork. But in practice, overwork alone rarely produces one — however little someone sleeps, a person who does not get nosebleeds does not get them. What a nosebleed does indicate is something else, and in certain circumstances it is doing useful work. This article covers nosebleeds in Korean medicine.
Scope of this article
This describes nosebleeds in otherwise healthy people, typically with acute illness. It does not describe nosebleeds arising from chronic conditions or in frail patients, which are read quite differently. Bleeding that will not stop after 15–20 minutes of pressure, recurrent nosebleeds in an adult, bleeding accompanied by easy bruising, or nosebleeds in anyone taking anticoagulants requires medical assessment rather than the reading below.

What Is Actually Happening
The bleeding comes from vessels in the nasal mucosa. What matters is where they sit: high in the body.
Veins in the legs rupture congestively — pressure accumulating downward against gravity. The nasal mucosa is the opposite case. There, bleeding happens because the speed of blood flow has increased and blood has crowded into the mucosa.
Where there is no chronic underlying condition, the commonest triggers are the inflammatory response to infection and simple external irritation.
Reading It Through KTM
In Korean Traditional Medicine (KTM), the traditional healing system of Korea also known as Hanbang (한방), faster blood flow means increased heat, or fire, in the body.
When heat arises it is transmitted to the blood in sequence. The reason a face flushes red with fever is exactly this — heat raising the speed of blood flow.
Which gives a simple reading: a nosebleed means heat was present.
Children make the point clearly. They get nosebleeds far more readily than adults, and children run hotter as a rule because they are growing. Growth requires heat, and a nosebleed is one of the ways that heat shows itself.
The Part That Is Useful
Heat raises the speed of blood flow, and it also carries an upward tendency. Fire rises. So when heat develops abruptly, the mucosa of the face — high in the body and richly supplied with vessels — flushes readily.
Now suppose bleeding occurs there and blood escapes outward. Heat leaves with the blood.
The Shanghanlun (상한론 傷寒論), the classical text on cold-damage and acute infectious disease that Korean medical students study, notes that when a nosebleed occurs in the course of such illness, the outlook is generally favourable.
So where illness has developed in the face, head or respiratory tract and heat has gathered there — during inflammation or an immune response — a nosebleed frequently coincides with the heat resolving and the illness clearing on its own.
It is the same action as sweating out a cold after eating something spicy. Different exit, same principle.
With children this is easy to observe: a nosebleed, and afterwards the fever comes down or some minor complaint clears. And the inverse holds too — children who are low in immunity and vitality get fewer nosebleeds, and their illnesses drag on longer.
What This Does Not Cover
I have written this about relatively healthy people with acute illness, because that is the case where a nosebleed carries a positive meaning worth knowing about.
Nosebleeds arising from long-standing disease, or in someone who is frail, must be read differently. There the bleeding is not heat finding an exit; it is closer to something failing to hold. That is a separate discussion.
And the practical boundaries are worth restating. Bleeding that will not stop with 15 to 20 minutes of steady pressure, nosebleeds that recur frequently in an adult, bleeding alongside easy bruising or bleeding elsewhere, or any nosebleed in someone on anticoagulants — these need medical assessment. The reading above is not a reason to wait.
Summary
Overwork gets the credit, but heat is what produces a nosebleed. In a healthy body meeting an acute illness, the bleeding is heat taking the nearest way out — which is why the classical texts treat it as a good sign rather than a bad one.
The child who bleeds and then recovers is not lucky. The bleeding was part of the recovery.
