Pancreotonia is the constitution everyone thinks of as running hot. It is also, in my clinical experience, among the most common to arrive complaining of cold hands and feet. That is not a contradiction — and understanding why explains something important: the warming foods these patients reach for tend to make it worse. This article covers cold hands and feet in Pancreotonia.

Everyone Is 36.5 Degrees
Start with the fact that makes the rest legible.
Patients who feel the cold badly, whose hands are cold and whose lower abdomen is cold, have a body temperature of about 36.5°C — the same as everyone else, absent infection.
Image them with infrared and the cold areas they describe do show as cold. But other regions show as comparatively hot. The total heat in the body is broadly similar from person to person.
Which means the words are doing something other than what they appear to. Cold and hot, as complaints, describe a relative sensation produced by the concentration or stagnation of qi and blood — not a deficit of heat overall.
The Two Axes of Pancreotonia
When Pancreotonia is taught in Korean medical schools, two aspects are emphasised.
- Disease arising from heat inside the body, as stomach heat increases
- Disease arising from cold on the outside, as the kidney weakens
Both belong to the same constitution. Most of the confusion around cold extremities here comes from attending to only the first.
Why the stomach is so hot
Stomach heat is the source of digestive power and among the hottest areas of the body. The classical description is vivid: food enters, passes through the stomach, and comes out resembling porridge — melted, in effect, by the heat there. Gastric acid is understood as that heat expressing itself.
Of the Sasang types, the Soyangin group is the one where heat is emphasised most. So physicians and patients alike tend to file Pancreotonia and Gastrotonia under “people with a lot of heat,” and there really are patients here whose fevers run fierce.
And why something has to be short
But the body is still 36.5°C. If the stomach holds that much heat and energy, some other region is short of qi and blood — and in this constitution that region is the kidney.
In Korean Traditional Medicine (KTM), the traditional healing system of Korea also known as Hanbang (한방), the kidney covers far more than the urinary system, taking part in excretion, metabolism and circulation throughout the body. Among the common symptoms of reduced kidney function are cold hands, cold feet, and a cold lower abdomen.
Read against the Eight Constitution Medicine (ECM) hierarchy, this is exactly what one would expect: Pancreotonia runs pancreas > heart > liver > lung > kidney. The kidney sits at the bottom.
In ECM, ‘strong’ does not mean ‘safe’ — and here the reverse also holds. A constitution defined by heat can present as the coldest person in the room.
How This Differs From the Soeumin Version
Any constitution can develop cold extremities for various reasons. But in my clinical experience Pancreotonia is among the most frequent and the most readily affected — because qi is congenitally concentrated in the interior and the stomach, leaving the lower abdomen and the extremities to run cold.
The water constitutions get there differently. In Renotonia and Vesicotonia, cold hands can arise because the stomach itself is cold. But the Soeumin types carry abundant qi in the kidney, so the pattern differs.
The distinguishing feature is practical and easy to ask about:
- Soeumin: the coldness resolves when stomach heat is adequate. It tends to clear after a good meal, or when digestion is going well and the person is eating in proportion to their strength.
- Pancreotonia: the coldness is more chronic and frequently unrelated to meals.
Ask whether eating helps. The answer sorts the two.
The Trap
Here is where this matters most.
A Pancreotonia patient with cold hands concludes, reasonably enough, that they are short of heat — and starts taking heating foods. Red ginseng. Black goat. Ginger. Chicken. Duck.
These add heat to the stomach and to nowhere else. Stronger stomach heat draws still more qi to the interior, and in many cases the cold hands get worse rather than better.
It is one of the clearest examples in ECM of a sensible-seeming inference producing the opposite of what was intended. The sensation is cold, so warmth seems indicated. But the sensation was never a report about total heat.
What Actually Warms the Hands
Eat foods that cool the interior while sending qi toward the kidney. That is the combination — not warming, and not simply cooling either, but cooling in a way that redistributes.
Readers who followed this site’s series on burning hands and feet will recognise the principle: cold does two things — it cools heat, and it drives qi downward. In Pancreotonia, whose kidney is most recessive, both actions help. That is why cool food and cold water suit this constitution while they harm the water types.
Cooling foods, fresh vegetables and fruit. Whole-body cold immersion remains the exception, since fire that cannot disperse outward withdraws inward instead.
And the hurry
A hurried mind increases heat in the heart and in the stomach.
Which gives this constitution something genuinely useful: Pancreotonia patients who manage their impatience well often improve on their own, and respond well to treatment. The temperament is not incidental to the physiology here. It is part of what is generating the heat that leaves the hands cold.
Before Assuming It Is Constitutional
Persistently cold or discoloured extremities can also arise from Raynaud’s phenomenon, thyroid disease, anaemia and circulatory conditions. Where the change is new, one-sided, or accompanied by colour changes or pain, that warrants medical assessment before any constitutional reasoning is applied.
Summary
The heat is not missing. It is in the stomach, and the hands are simply where it is not.
Which is why the treatment runs the opposite way from what the symptom suggests — cool the middle, send qi downward, and slow down.
