Why Different Clinics Give You Different Constitutions, Part 1: How Sasang Diagnosis Works

“One clinic said I was Taeeumin, another said Soeumin.” I hear this often. The reason is simple to state: constitutional diagnosis is not like the tests we usually think of, such as a blood test or an X-ray. There is not yet any visible or measurable value that tells you someone’s constitution. This first part explains how Sasang constitution is diagnosed in practice, and why different clinics can reach different answers. Part 2 covers Eight Constitution Medicine (ECM).

In Summary

  • No current test can photograph or measure constitution. Attempts using body shape, facial contour or organ size have not produced a confirmed visible marker.
  • Sasang diagnosis runs in three steps: a constitutional questionnaire, a clinical consultation, and treatment with a constitutional herbal formula.
  • Questionnaires give tendencies (for example 60% Taeeumin, 30% Soyangin), not a single answer; typical cases can be identified this way, many cannot.
  • Confirmation comes from the response to constitutional herbal medicine — which takes time, and occasionally a non-matching formula works well for a short while.
  • Taeyangin is especially hard to identify: because its liver is weak, there are hardly any medicinal herbs that suit it in the first place — and so hardly any formulas to test with.

Constitution Is Not Something We Can See Yet

I believe a day will come when technology lets us image Sasang or ECM constitution directly. Until then, nobody can see it. There have been attempts to measure it from body shape, facial contour, or the shape and size of organs, but none has produced a visible marker that confirms constitution.

How Sasang Constitution Is Diagnosed Today

In Korean Traditional Medicine (KTM), the traditional healing system of Korea also known as Hanbang (한방), Sasang diagnosis usually proceeds in three steps.

1. The constitutional questionnaire

The most important meaning of constitution is a tendency in how a person falls ill. So by putting together the illnesses someone gets often, the tendencies that show when they are worn out, and their usual temperament, a Sasang constitution can be inferred.

By its nature, a questionnaire does not hand you one correct answer. The last Sasang questionnaire I took, as I recall, gave something like: Taeeumin tendency 60%, Soyangin tendency 30%, Soeumin tendency 20%. People who are textbook examples of a type can be identified by questionnaire alone. Most people are not textbook examples.

2. The consultation

A questionnaire cannot ask every important distinguishing question, so the practitioner works out the constitution through a consultation built around the patient’s current problems.

3. Treatment

Sasang treatment relies mainly on constitutional herbal formulas. Based on the constitution and condition worked out from the questionnaire and consultation, the practitioner prescribes a constitutional formula and watches the response — and that response is what confirms the constitution.

Where the Difficulties Lie

  • The first answer is a starting point. Questionnaire and consultation alone cannot pinpoint Sasang constitution accurately from the outset; a confirmation process is needed.
  • Confirmation can take time. Sometimes the only way to know is to finish a course of medicine.
  • A non-matching formula can still help briefly. It is uncommon, but a formula for another constitution can work well in the short term, which muddies the picture.
  • Herbal medicine is the only confirming tool. Sasang medicine has no other way to check the constitution.
  • Taeyangin is especially hard. Because the Taeyangin constitution’s liver is weak, there are hardly any medicinal herbs that suit it to begin with. With almost no suitable herbs, there are almost no formulas either — so there is little to confirm the constitution with through treatment response.

Why This Is Not a Reason to Give Up

Living things never stop changing, so even imaging and blood tests do not always give consistent results. Sasang constitution reveals itself through treatment and clinical expression, but it cannot be seen, so there is no test that confirms it. It has to be established through consultation and constitutional herbal medicine, which takes time and effort — and on rare occasions the response to a medicine may have nothing to do with constitution.

KTM physicians keep working to make Sasang diagnosis more consistent and accurate, and I am sorry for the confusion it can cause patients. But constitution describes an imbalance in function — in energy — which current technology cannot observe directly. Please bear with that, and take the time to confirm your constitution properly. I think of constitution as an important owner’s manual for the body, worth the time and effort it takes to learn.

Summary

Sasang constitution cannot yet be measured. It is inferred from a questionnaire and consultation, then confirmed through the response to constitutional herbal medicine — a process that takes time, occasionally misleads, and is hardest for Taeyangin. That is the main reason different clinics can give different answers. Part 2 looks at how ECM approaches the same problem.
