Burning Hands and Feet in Pulmotonia: Heat That Leaves Before It Settles

Heat in the palms and soles is an interior complaint. Pulmotonia, meanwhile, is a constitution with a strong lung function that sends heat and energy outward. So burning hands and feet tends to stay comparatively mild here and rarely hardens into a chronic condition. In Eight Constitution Medicine (ECM), “strong” does not mean “safe” — but in this particular case the dominant side is doing useful work as an exit.

Why It Stays Mild in This Type

Korean Traditional Medicine (KTM), the traditional healing system of Korea also known as Hanbang (한방), sorts heat in the hands and feet into three broad routes: a kidney that is weak or has qi concentrated in it; a pancreas-stomach axis that is deficient or excessively strong and has accumulated heat; and heat accumulated in the heart.

Pulmotonia’s hierarchy runs: lung and large intestine > pancreas and stomach > heart and small intestine > kidney and bladder > liver and gallbladder. The dominant position belongs to the lung and large intestine, the most recessive to the liver and gallbladder.

And burning palms and soles is a symptom tied to the body’s interior. Since this constitution’s strong lung disperses heat and energy outward, heat has difficulty pooling inside in the first place.

The Sweat Comes Before the Heat Can Settle

In the years I spent in clinical practice, the Pulmotonia patients I met would sweat readily once heat appeared, so the duration of the heat was short.

The exit is already open. This contrasts with Hepatonia, where the same symptom runs strong and lasts — Hepatonia stores in the interior and Pulmotonia releases, so the heat spends a different amount of time in the body.

Which means that when burning hands and feet do appear in Pulmotonia, the more useful question is not what the heat is, but why the exit is blocked right now.

First Route: A Weakened Liver or Kidney

Weakened here does not mean a kidney damaged to the point of requiring dialysis. It refers to functional deficiency as KTM understands it — a reversible degree that recovers.

In this case, sufficient sleep and food that suits the constitution are enough to restore it.

  • Leafy greens — napa cabbage, lettuce, cabbage, water dropwort, spinach, squash leaf, bracken, ashitaba, kale, mugwort, crown daisy
  • Plant-based ferments — kimchi, doenjang
  • Most fish — blue-backed fish (saury, herring, anchovy, hairtail, mullet, gizzard shad) are good, but remove the fatty portions
  • Seafood — shrimp, oysters, sea cucumber, crab, crayfish, pufferfish
  • Shellfish — hard clam, blood cockle, short-necked clam, and abalone among others
  • Fruit — green grapes, kiwi, peach, banana, cherry, strawberry, Korean melon, pineapple

Second Route: Sensitivity Turned Up

Pulmotonia has several sub-patterns, and among them is a sensitive one that can come with headaches or feverishness.

Staying too long in the world of imagination and invention leaves the body unable to keep pace. Stepping out of it to rest is part of the treatment.

This is not a claim that the constitution determines temperament or occupation. It is a claim that when sensitivity intensifies in this type, there is a route by which that state becomes heat.

Third Route: Liver Heat From Unsuitable Food

No Pulmotonia patient avoids every unsuitable food indefinitely. But when the heat is bad, that is the period to pay attention.

  • All meat — beef, chicken, pork, dog, duck, goat, lamb, venison, egg yolk, ham, sausage
  • Dairy — milk, yoghurt, cheese, butter
  • Wheat flour products — ramyeon, jajangmyeon, noodles, bread, biscuits
  • All oils — sesame, perilla, walnut, brown rice, cooking oil, fried food, stir-fried food

There is one more thing to read here. Heat appearing at all can also signal that liver and kidney function is being spent. The liver is the most recessive organ in this constitution, so that signal is not a light one. Caught early, though, it is a function that recovers with management.

Summary

Pulmotonia does not hold heat well, which is usually a mercy. Burning palms and soles here are less a disease of the extremities than a note that something upstream — the liver, the kidney, or a nervous system running too hot — has been drawn down further than usual.

The heat will leave on its own. The question is what let it gather.
