Headache is classed as a disease, but it behaves much more like a symptom — the end point of something else. That is why a constitution-by-constitution account is useful: it moves the question from what kind of headache is this to where in this particular body is it coming from. In the Colonotonia constitution (금음체질), the answer is unusual. Colonotonia headache generally does not start in the head. It starts in the large intestine.

The Headache Organs Are Not This Type’s Weak Point
In Korean Traditional Medicine (KTM), the traditional healing system of Korea also known as Hanbang (한방), the organs most often implicated in headache are the pancreas-spleen, the kidney and the liver. Fire generated in quantity, or qi (氣) failing to reach the head in sufficient supply, is where most headache patterns begin.
Colonotonia’s organ hierarchy runs: lung and large intestine > kidney and bladder > heart and small intestine > pancreas and stomach > liver and gallbladder. Looking at that order, the classical headache organs are not where this constitution characteristically breaks down. On paper it should be a low-headache type.
In practice it is not. And the reason is the organ at the top of the list.
A Reactive Large Intestine Tightens the Whole Body
Colonotonia is the one constitution in which the overactivity of a strong bowel is the established clinical centre — this is settled doctrine within ECM rather than an inference. The large intestine here is powerful, and it becomes reactive very easily: greasy food, dairy and wheat can set it off within hours.
This is the plainest illustration of a principle that runs through all of ECM. In ECM, “strong” does not mean “safe” — the dominant axis carries the most qi and tips most easily into excess. Colonotonia’s trouble comes from its strongest organ, not its weakest.
What matters for headache is what happens next. When the large intestine becomes irritated in this type, the disturbance does not stay local. It transmits outward: muscle tissue tightens, other organs go on guard, and the tension climbs the back, the shoulders and the neck until it reaches the head. What the patient experiences is a headache. What produced it was a bowel.
In the years I spent in clinical practice, Colonotonia patients often arrived having had their necks imaged, their posture corrected and their eyes tested, all to no effect. The investigations had been looking in the wrong place.
A Sensitised Baseline
There is a second layer. Colonotonia is one of the four sympathetic-tense constitutions — Pulmotonia, Colonotonia, Renotonia and Vesicotonia — and in this type the sympathetic bias is pronounced. The body sits at a raised baseline of alertness and reactivity, on average, most of the time.
A body already held at tension does not need much to cross into pain. So headache comes readily in this constitution, and, more to the point, it tends to become chronic rather than episodic. The pattern I saw was not one dramatic headache a month but a low, persistent, familiar one that the person had stopped mentioning.
Why Sweating and Deep Massage Backfire
Here is the part that most needs saying, because the instinct runs the other way.
When the head and the back of the neck feel tight, the natural response is to loosen them — a hot bath, a sauna, a session of sweat-raising exercise, or a firm pair of hands working the neck. For a Colonotonia, all of these produce genuine relief, and the relief is short-lived.
The reason is constitutional. Sweating and bloodletting are dispersing measures suited to the four parasympathetic-tense types — Hepatonia, Cholecystonia, Pancreotonia and Gastrotonia. For the sympathetic-tense four, they are depleting. The Colonotonia patients I saw who managed headache by sweating it out felt better that evening and worse across the season. Repeated often enough, the practice fixes the headache in place rather than clearing it.
The same holds for strong massage. Forceful pressure on an already-guarded system provokes more guarding once the immediate effect wears off. Gentle work is a different matter; force is what causes the problem.
Managing Colonotonia Headache
Start with the bowel, through diet. Since the tension usually originates in the large intestine, the constitutional diet is not a general wellness measure here — it is the specific treatment. The plant-and-sea pattern that suits Colonotonia calms the large intestine directly, and as the bowel settles, the whole body releases. Patients often noticed the headache easing before they noticed the digestion improving.
Take substances seriously. This constitution is affected more than most by unsuitable drugs, foods and alcohol. Where another type might absorb an off week without consequence, a Colonotonia registers it. More caution is warranted here than the average person needs, and that is a statement about the design, not about willpower.
Accept the sensitivity rather than fighting it. The Metal-dominant constitutions tend toward perfectionism and toward orienting their attention outward — weighing how things look to others, anticipating what other people need. Much of Colonotonia’s capability comes from exactly that. But it is also a way of holding the body at tension all day long.
So the advice I would give a Colonotonia with a chronic headache is partly not medical. Set the outside gaze down for a while. Give the time and the affection to the person inside instead. In this constitution that is not a consolation prize — it is upstream of the symptom.
One boundary: a headache that is sudden and severe, unlike your usual pattern, accompanied by fever, neurological signs or weakness, or steadily worsening, needs conventional medical evaluation first. Constitutional reasoning is for the recurrent, unexplained headache.
Dietary Reference
Beneficial: green leafy vegetables — napa cabbage, lettuce, cabbage, water parsley, perilla leaf, spinach, pumpkin leaf, bracken, angelica keiskei, kale, mugwort and crown daisy; plant-based fermented foods such as baechu kimchi and doenjang; most fish, with blue-backed fish (saury, herring, anchovy, hairtail, mullet, gizzard shad) especially good, though the oil should be removed; seaweeds including gim, miyeok, dashima and parae, taken with care if there is stomach or skin trouble; shellfish including hard clam, ark shell and abalone; green grape, kiwi, tangerine, orange, cherry, tomato and pineapple; rice, buckwheat, mustard, cocoa, quince tea, cucumber and crayfish. For exercise, low-sweat forms suit this type — swimming above all, along with calisthenics, walking, meditation and yoga.
Harmful: all meat — beef, chicken, pork, duck, goat, lamb, venison, egg yolk, ham and sausage; dairy including milk, yogurt, cheese and butter; wheat products such as ramyeon, noodles, bread and biscuits; all oils, including olive oil, and fried or stir-fried food; pear, apple, red grape, watermelon, melon and persimmon; root vegetables including radish, carrot, garlic, onion, lotus root, deodeok, bellflower root, bean sprouts and burdock; eel, loach, catfish, croaker, pollack, carp, snakehead, shrimp, oyster and sea cucumber; nuts such as chestnut, pine nut, ginkgo, walnut, peanut and almond; all mushrooms; pumpkin, gourd, coffee, chili, sorghum, soybean, adlay, yam, kudzu, sweet potato and artificial seasonings. Heavy sweat-raising exercise is unsuitable.
The list does not have to be kept perfectly. Keep to it as far as is realistic — during a headache period, a little more strictly.
In Summary
Colonotonia headache is a message from the large intestine, delivered by way of the muscles. Treat the head and it returns; settle the bowel and loosen the grip the mind keeps on the outside world, and it tends to go quiet. The one thing not to do is sweat it out.
