Pancreotonia and Gastrotonia Skin: Stomach Heat With Nowhere Else to Go

Skin trouble in the earth constitutions does not begin at the skin. It begins as heat that has accumulated in the stomach and found the face as its exit. In Eight Constitution Medicine (ECM), “strong” does not mean “safe” — the dominant axis carries the most qi and tips most easily into excess. In Pancreotonia skin, that axis is the pancreas and stomach, and the trouble on the face is the excess showing itself.

The Face Belongs to the Stomach

In Korean Traditional Medicine (KTM), the traditional healing system of Korea also known as Hanbang (한방), the face is governed chiefly by the Yangming channels, which belong to the stomach and the large intestine. When heat accumulates in the stomach, the face is where that heat is directed.

This is why treating acne as a surface condition produces such repetitive results. As long as heat keeps rising, work done at the surface stays at the surface. The lesion is on the face; the fire is further down.

How the Heat Is Made in Pancreotonia

Pancreotonia’s organ hierarchy runs: pancreas and stomach > heart and small intestine > liver and gallbladder > lung and large intestine > kidney and bladder. The pancreas-stomach axis is most dominant and the kidney-bladder axis most recessive.

When the kidney is weak, its counterpart — the pancreas and stomach — can grow excessively strong. That force condenses, and fire accumulates in the stomach.

The order here matters more than it first appears. The illness does not settle in the weak kidney. The weak kidney pushes the strong axis higher, and the problem appears on the strong side. This is what “strong does not mean safe” means in practice. People reason that a weak organ must be the vulnerable one, and in ECM the reasoning runs the other way more often than not.

What to Take Out

Everything spicy. Spring onion, onion, ginger, mustard, pepper, curry, garlic. Chicken, dog, lamb, goat. Glutinous rice, brown rice, potato, corn. Sesame oil, sesame, cinnamon, jujube. Apple, tangerine, orange, tomato.

On the suitable side: pork, egg white, beef. Barley, red bean, soybean, Job’s tears. Most fish and shellfish. Korean melon, pear, watermelon, kiwi. Perilla seed and perilla oil, cucumber, napa cabbage, lettuce, radish, water dropwort.

Not cooking food to death and not eating it scalding hot belong to the same principle. So does one item that surprises people: ginseng does not suit the earth constitutions. A herb with a reputation for restoring vitality adds heat to a body that already has too much of it. Reputation is not constitution.

The Earth Types Should Sweat — and This Is Where the Advice Splits

ECM divides the eight constitutions into two groups of four by autonomic tendency. Hepatonia, Cholecystonia, Pancreotonia and Gastrotonia are parasympathetic-tense, and dispersal through sweating benefits them. Pulmotonia, Colonotonia, Renotonia and Vesicotonia are sympathetic-tense, and sweating and bloodletting damage them.

The earth types belong to the first group. Sweat-producing exercise and warm baths suit them. Swimming and cold rubs, which chill the body, do not. The lower abdomen should be kept warm.

On the surface this looks like a contradiction. If heat is the problem, why add heat? Because the problem is not the quantity of heat but the fact that it is trapped. Sweat is an exit for heat that has nowhere else to go. Cooling the surface without opening an exit drives it inward.

The Mind Makes the Heat

Pancreotonia has three signature conditions and only three: early-onset diabetes, hwa-byung (火病), and difficulty conceiving. Skin is not among them, and I want to be careful not to inflate it into one. Signature conditions are over-represented, not exclusive, and the reverse holds too — a condition’s absence from the list does not mean this type never sees it.

But hwa-byung and the skin share a root. A hurried, pressed, unexpressed state of mind generates heat; the heat becomes trapped in the stomach; the stomach sends it to the face. In the years I spent in clinical practice, patients of this type would describe their diet in detail and their pace of life not at all — and the pace was often doing more work than the diet.

So in this constitution, slowing down is not adjacent advice. It is part of the prescription.

How Far We Can Honestly Speak About Gastrotonia

Gastrotonia is the rarest of the eight, and the clinical record behind it is correspondingly thin. What is settled is that the stomach is strong and the kidney weak, and that its signature conditions are penicillin anaphylaxis and vitiligo.

Past that point I am reasoning rather than reporting, and I would rather say so than let inference pass as observation.

In my reading, the route by which stomach excess produces heat is the same in Gastrotonia as in Pancreotonia. What differs is the recovery side. The kidney is not as recessive here as it is in Pancreotonia, and lung function — which disperses heat — sits higher in the order. That combination suggests heat both accumulates and clears on a different timetable. The dietary principle is unchanged: avoid what heats the stomach, choose what cools it.

This is an interpretive framing drawn from the hierarchy, not a clinical finding. If you are of this type and your experience runs against it, your experience is the better evidence.

Summary

In the earth constitutions, the face reports on the stomach. Heat gathers on the dominant axis, the Yangming channels carry it upward, and it surfaces where everyone can see it. The work happens in three places at once: take out what heats the stomach, open an exit through sweat and warm bathing, and slow the mind that keeps generating the heat in the first place.

Treating the face alone leaves all three untouched.
