In the two articles before this one, skin care for the wood and earth constitutions converged on the same instruction: sweat, take warm baths, open an exit for trapped heat. In the water constitutions that instruction reverses. Saunas, hot rooms and long warm soaks work against these types. This is the point where the same complaint yields opposite prescriptions — and it is also the clearest answer to why identical skin advice produces such different results in different people. Renotonia skin and Vesicotonia skin are decided somewhere else entirely: at the stomach.

The Four-and-Four Divide — Where the Advice Flips
Eight Constitution Medicine (ECM) divides the eight constitutions into two groups of four by autonomic tendency. Hepatonia, Cholecystonia, Pancreotonia and Gastrotonia are parasympathetic-tense, and dispersal through sweating benefits them. Pulmotonia, Colonotonia, Renotonia and Vesicotonia are sympathetic-tense, and sweating and bloodletting damage them.
So the familiar instruction to sweat the toxins out does not apply here. Hot rooms, saunas, long warm soaks and hard aerobic work drain this type rather than clearing it. What suits instead is exercise that does not raise much sweat — swimming, walking, calisthenics, yoga, meditation.
It is worth sitting with how large this divergence is. Two people with the same complaint on the same face receive opposite instructions, and both instructions are correct. Constitution is not a refinement on general advice. It is often the thing that decides its sign.
The Shared Ground: A Recessive Pancreas and Stomach
In both Renotonia and Vesicotonia the pancreas and stomach are the most recessive organs. Cold food, raw food and overeating land directly as burden.
Unsuitable: ice, ice cream, cold drinks, cold noodles, barley tea. Raw fish and uncooked vegetables. Pork and egg white. Barley and red bean. Clams, shrimp, oysters, sea cucumber, squid. Korean melon, strawberry, banana, persimmon.
Suitable: chicken, lamb, goat, beef. Glutinous rice, rice, brown rice, potato. Laver, sea mustard, kelp. Spring onion, onion, ginger, mustard, pepper, curry. Apple, tangerine, pear. Scorched-rice tea, sesame oil, cinnamon, jujube. Everything is eaten warmed through.
Why Fine Skin Is Common in the Water Types
In the years I spent in clinical practice, I often saw fine skin in Renotonia and Vesicotonia patients. It is not a coincidence, and the hierarchy explains part of it.
First, water qi is abundant. In Vesicotonia, when the stomach runs moderately cool, a pale complexion can come as a birthright. People with a great deal of stomach heat, by contrast, tend toward a red or darkened face.
Second, these types eat modestly. When heat accumulates in the stomach and large intestine it rises to the face as trouble — but with the pancreas and stomach most recessive, these constitutions cannot eat much in the first place. The route by which that heat accumulates barely operates. The lesson generalises past constitution: eating modestly favours the skin.
Third, recovery is good. Skin repair depends on the nourishing action of jing (精) and blood, and both are ample in the water types. What is short is qi — the driving force that moves them.
None of which means care is unnecessary. Break the conditions — eat something cold, strain the stomach — and this type develops skin trouble like any other. And as the next section shows, when the lung side runs excessively strong in Renotonia, the result can be severe skin disease rather than fine skin. In ECM, “strong” does not mean “safe” — the dominant axis carries the most qi and tips most easily into excess.
So what you manage in these constitutions is not the face. It is the stomach.
Renotonia: The Side That Resembles Colonotonia
Renotonia’s organ hierarchy runs: kidney and bladder > lung and large intestine > liver and gallbladder > heart and small intestine > pancreas and stomach. Colonotonia’s runs: lung and large intestine > kidney and bladder > pancreas and stomach > heart and small intestine > liver and gallbladder. The two dominant axes are the same pair — lung and kidney — only in reversed order, and the two constitutions share attributes as a result.
There is one practical implication. Renotonia’s lung side can also run excessively strong, and when it does, skin disease can present as severely as it does in Colonotonia. Atopic dermatitis is best known as concentrated in Pulmotonia, but it appears in Colonotonia and Renotonia at rates that are not negligible. When skin becomes the presenting problem in Renotonia, reducing meat is a useful direction.
The difference from Vesicotonia comes out here as well. Vesicotonia has weak digestion, so the governing variable is quantity. Renotonia digests more robustly — many eat well — so both quantity and kind require management.
Vesicotonia: Not Letting the Stomach Get Cold Is the Whole of It
Vesicotonia’s organ hierarchy runs: kidney and bladder > liver and gallbladder > heart and small intestine > lung and large intestine > pancreas and stomach. The stomach is cold and weak, illness starts there, and the type can progress to gastroptosis. Gastroptosis matters because it creates a structure that drives overeating, which makes the original problem worse — a loop rather than a line.
So this constitution’s principles compress to two: eat moderately, eat warm. When the stomach is comfortable most of the rest follows. When the stomach breaks down the skin goes with it, and when the stomach is restored the skin returns. Recovery itself tends to be quick, for the reason given above — jing and blood are sufficient here, and what is lacking sits on the qi side.
When Tonics Are Used
Ginseng and honey suit both Renotonia and Vesicotonia, and Sipjeondaebotang (十全大補潟) is a well-matched formula for these types. Strong warming materials such as ginseng, deer antler and aconite still require care over timing and dose — a correct direction is not an argument for a heavy hand. Reishi, aloe, abalone and vitamin E are generally held to run the other way.

Summary
Acne is fire in the skin, and where that fire can be released is decided by constitution. The wood and earth types release it through sweat. The water and gold types do not have that exit — they protect the stomach and the large intestine so the fire is never built in the first place.
For the water types this means the sauna is not neutral, the cold drink is not neutral, and the large late meal is not neutral. It also means the skin tends to answer quickly once the stomach is looked after, which is a better bargain than it sounds.
